About
There are never enough staff in a medical practice, physician or non-clinical, to handle all the work to be done — from reviewing patients’ test results, to calling patients to discuss them, to answering patient calls about scheduling and clinical questions, to calling patients to check if they’re taking meds rights. Simbie is the clinically intelligent AI voice agent that automates and expands all of this work, saving practices time and money while adding patient touchpoints that save lives, but that practices can’t afford human staff for. We're the first platform executing AI workflows requiring real clinical judgment, directly in the EHR.
We're YC-backed, distribution is proven, and we're signing customers faster than we can currently bring them live. That's the problem we need you to solve.
The Role
You'll own the full technical vision and be genuinely hands-on — architecture decisions in the morning, code review in the afternoon. You'll partner directly with a physician and a clinical operator who've already proven distribution. Not a figurehead: you build, you ship, you talk to customers.
- Builder first. You sketch the MVP while the physician is still describing the workflow. Fast, demo-driven, unafraid.
- Customer-close. You feel physicians' pain directly and translate it into product. Strategy and code — both.
- Humble and collaborative. Strong conviction, no ego. You thrive partnering with a medical team that's already winning.
What You'll Own
- Engineering team — hire and lead from scratch, set the culture
- Data strategy — build the first between-visit clinical data layer; use it to improve models over time
- EHR integrations — repeatable framework across EHRs; strategic calls on prioritization and TAM
- Technical roadmap — architecture, AI/ML strategy, infrastructure
What We're Looking For
- 7+ years engineering, 3+ in technical leadership; startup experience essential
- Deep AI/ML: LLMs, RAG, fine-tuning, agents, vector databases
- Python primary; React / TypeScript a plus
- EHR integrations (FHIR, HL7) and healthcare data — strongly preferred
- Shipped production AI systems; comfortable with cloud infra and MLOps
- Bonus: voice AI, multi-modal models, prior founder/founding engineer experience

About Simbie AI
Simbie AI is the AI agents’ system for healthcare follow up, ensuring patients get to their next visit. Today 30% of return visits don’t happen because patients get lost in follow up logistics, costing doctors 10% of revenue. Our clinically intelligent, EHR-integrated AI agents proactively reach out to patients and answer their messages, via text and voice, at vulnerable touchpoints for loss to follow up - after new diagnoses, new meds, or diagnostic tests are ordered - and close the loop on every request, documenting directly in the medical chart, until the patient is handed off to their return visit.
Simbie is founded by Natalia Khosla MD MSc, a physician from UChicago and Fulbright Scholar in Gender & Healthy Equity and Rachel O'Driscoll, a healthcare operations leader who ran US product operations at Carrot Fertility and launched a tech-enabled clinic at Cayaba Care. Natalia and Rachel met during their undergrad days at Yale and have been best friends for 15 years.
